This particular research study is predicated upon addressing a significant healthcare issue that entails reducing the unacceptable rates of 30 day hospital readmissions post Coronary Artery Bypass Graft (CABG) surgery. The impetus for this study is to develop a competent teaching program that can be implemented early during the patient's discharge following surgery wherein the use of adequate discharge planning is provided to offer in-depth knowledge of the postdischarge experience to patient, which will better prepare them for the life changing events that occur post-recovery. The hypothesis is that this will reduce the high rates of re-hospitalizations and deaths. CABG discharge planning programs that are more efficiently aligned with recovery resources will facilitate the necessary programs that allow individual patient needs to be accessed, which will provide patients' with a more efficient timeframe to manage their CABG post surgery health issues.

To accomplish this goal, the research paper suggests that a technological solution known as the HeartCare system is utilized. This internet-based computer technology will facilitate easier healthcare delivery of individually tailored recovery information, which will provide patients with increased peer support, and nurse-patient communication to patients' homes.

In reference to principles and laws that govern the life-process, CABG surgery represents one of the most performed surgeries in any hospital setting, which mandates that healthcare providers are able to provide the necessary information and care that is required for a reduction of the high numbers of re-hospitalizations of CABG patients upon discharge. Nursing actions or processes that will facilitate positive change entail nurses being the healthcare providers who teach patients the appropriate information and steps to induce positive changes in their post-surgery outcomes. By starting the education process during the beginning of their post-surgery healing process while the patients are still in the hospital, which must continue throughout the rest of their post-recovery wherein technology is used to interact with patients in their home environments.
Orem's Self-Care Deficit Theory will be the grand nursing theory used in this particular research summary as it dictates that patients must become self-reliant, and be able to provide self-care as well as others in their family who when necessary should possess the skills and the ability to provide care to their loved ones. This theory acknowledges that nursing is contingent upon an interaction between two or more people, which requires successfully meeting universal and development self-care requisites to improve care delivery and reduce the propensity for ill health post CABG surgery.
