In a situation wherein the security of a prison institution has become compromised, it's imperative that the leadership within this facility take a holistic approach toward resolving any issues that may be causing security lapses within the prison. Prisons are supposed to ensure that those who have committed criminal acts in society are incarcerated and unable to further commit future criminal behavior. This is especially imperative for violent offenders as any security breaches with these high-risk prisoners could endanger the staff, other prisoners, and the general public. Therefore, in a prison that has experienced four escapes, assaults among inmates, and several assaults on the staff requires an entire overview of the policy and regulations at the prison.

The first and most important function to complete is to ensure that measures are taken immediately to shore up more security during the interim time that it takes to conduct an investigation. Therefore, I would hire more personnel and consult with security professionals within the industry to ascertain how I could instill more security measures to negate any potential impending escape attempts. Obviously the security measures within the prison are lax because many inmates have escaped. Therefore, taking extreme measures to shore up security measures such as electric shock fences, the use of reinforced steel doors, and other security measures that are able to prevent inmates from leaving the facility is the first and most important measure to take.

More security guards will also be necessary for ensuring that inmates are guarded with the highest proportion of inmate to officer ratios possible to negate the attempts at escaping as deterrence by using a saturation strategy would also provide a deterrent to inmates potentially planning an escape. To obtain these objectives I would need resources and funds. In many prisons, resources are limited and if they are ran by the state, this situation is exacerbated. Therefore, I would have to convene a committee that produces a detailed report insinuating the significance of the problem to either private shareholders in a private prison or to state representatives in a state prison.
