Lacy is a 16 year old Caucasian cis-gender female student at a local university. She presented for counseling at the insistence of a professor who was worried about her mental health. Lacy lives with her parents and attends university as a PSEO student as she also completes her high school degree. Her academic record is outstanding, earning all A's and having received several awards for her accomplishments. In addition, she is a musician who plans to pursue education at an internationally known conservatory once she turns 18. Lacy's professor was worried about her due to her isolation within the classroom. Additionally, she has noticed Lacy has lost a significant amount of weight since the term began three months ago.

During the intake interview, Lacy reported she has no appetite and finds it very difficult to eat. Having more than one bite of food causes her to feel nauseous and uncomfortable for the next several hours. She is able to eat at home with her family, and is primarily affected at school. She discussed her social life as limited to two friends who are also musicians as well as her music teachers. She has never dated and while interested in men, has had limited friendships or interactions with anyone other than her two female friends. Her friends do not attend university and she has not formed any acquaintances on campus.

Lacy is an only child. Her parents are married and both work outside the home. Growing up, her mother worked part time for the first seven years of her life and spent the rest of her time helping Lacy attend various music performances and homeschooling her. She is also close to her maternal grandparents, who assisted her in getting to lessons. Her maternal grandfather was recently diagnosed with cancer and is not expected to live more than a year.

As a child she met all developmental milestones on time. She was often ill as a child and frequently stayed home from school due to stomach aches. In third grade she was "bored" and "nervous" in school so her parents started homeschooling her to support her rapid academic growth without having to introduce her to peers older than her. She showed an interest in piano at the age of two and started lessons when she was four. She has since switched to primarily violin, although still takes weekly piano lessons.

Lacy denied any frequent worries, has no thoughts or history of any self-harm or suicidal behavior. She continues to be passionate about her music, but has limited interests outside of her performance. When questioned on that, she stated she has no time between her studies and several hours of practice per day. She stated she is uncomfortable with her appearance, as she feels too "shy" to wear anything other than large sweaters and coats. In the summer she stated she wears mostly long sleeved button-up shirts and ankle-length skirts. She attended the first session wearing a wool skirt, boots, and a knee-length coat, which she did not remove. Her appearance was otherwise unremarkable. She had neatly brushed hair, which was left down and not styled. Her speech was slightly soft, but otherwise had a typical pace and tone. Her eye contact was slightly less than typical.

The Minnesota Multiphasic Personality Inventory (MMPI) is a tool utilised by psychologists to assess personality traits as well as psychopathology (Framingham, 2016), the purpose of which is to "primarily intended to test people who are suspected of having mental health or other clinical issues. " Through practice overtime, MMPI has become one of the most utilized psychological tests in the world since its publication in 1943 (Stanborough, 2020) by clinical psychologists Starke Hathaway and neuropsychiatrist JC McKinley of the University of Minnesota. It has been regularly updated for applicability and relevance and one of the easiest to deploy giving it reliability and validity.

One of the most common updates/version is MMPI-2 and is a self-report inventory with 567 true/false questions about the self. The answers, when compiled bring forth a picture of the test taker's personality and mental state which is used by clinicians and practitioners to determine symptoms or indications of mental illness or personality disorder.
