Chi-Square Test - prostate cancer

Hello, I have also attached this same problem in a word document.
Thank you.

Medical researchers followed 6272 Swedish men for 30 years to see if there was any association between the amount of fish in their diet and the incidence of prostate cancer. (The data are collected from an article in Lancet, June, 2001.) The data are found in the table below.

Fish
Consumption Total
Subjects Prostate
Cancers
Never/Seldom 124 14
Small part of diet 2621 201
Moderate Part 2978 209
Large Part 549 42

I am asking you for an analysis and your conclusions about whether you think there is a relationship between the amount of fish in the diet and the incidence of prostate cancer.
(Be sure to note that this data is not in the form you need it to analyze it. Convert it to a form so that a chi-square test can be done on it.)
