What is unclear however is if you have to provide such a test or to just provide answers to a and b as they pertain to your test. I think it is the latter but it is still good to create a 'sample test' that would feature your answers to a and b. As such I suggest the following outline to answer this question:

1. Explain your depression test - what strategy would you do? Is it a simple test?
2. Answer a here.
3. Answer b here.

Altogether it will be around 300 words or more. I have also included a sample short depression test based on this. I encourage you to also use the listed resources to further explore the topic in question. Good luck with your studies.
