    A cause and effect diagram, also known as an Ishikawa or "fishbone" diagram, is a graphic tool used to explore and display the possible causes of a certain effect. Cause and effect diagrams are a schematic means of relating the causes of variation to the effect of variation on the process. In other words, it is a drawing to organize the contributing causes to a problem in order to prioritize, select and improve the source of the problem. One possible example of how the fishbone diagram is used in healthcare is to determine the causes of adverse reactions to drug administrations.
