(a) Being accountability for continuous improvement by setting SMART objective quality measures to measure the improvement periodically and to improve the quality based on the incoming data from these measures. For example, let's say that the risk management plan included a questionnaire of patient satisfaction. Part of being accountable would mean using the research results to improve delivery. If the patient's were not satisfied and left unsafe, then accountability would be about fixing the problem.
