Based on the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR [APA]. 2000), Mr. A has a disorder associated with paraphilia. The essential features of a paraphilia are recurrent, intense sexually arousing fantasies, sexual urges, or behaviors generally involving 1) nonhuman objects, 2) the suffering or humiliation of oneself or one's partner, or 3) children or other non-consenting persons that occur over a period of at least 6 months (Criterion A). Some of the paraphilic fantasies occur during stress. Mr. A's disorder is classified as Transvestic Fetishism and involves fantasies such as: (a) cross dressing, (b) sexual arousal accompanied by images [such as the desire to be female, and (c) wearing women's garments with no attention to genitalia.
