In one paragraph, explain the need for change in current healthcare practice setting, a previous practice setting, or in an education practice setting.
The need for change within current healthcare settings is predicated upon a lack of care coordination that currently exists among providers. Because of this disparity between providing coordinating care and coordinated care, much waste occurs as well as potential mistakes that impact patient outcomes, which often result in healthcare providers being forced to repeat tests and procedures, patients suffering hospital related injuries and infections, unnecessary re-hospitalizations, and more than necessary difficult transitions back into their lives after leaving the hospital. The change within the healthcare setting must account for these issues while increasing the amount of nurses within the organization as more patients will need care while nursing shortages are growing significantly higher.
