In my organization, measurements are based on student success in the classroom and standardized tests. If students aren't excelling, needs assessments are used to ascertain the student's needs as well as if the teacher is lacking in areas of instruction or competency. Students and teachers who aren't "measuring' up to the standardized testing measurements must receive additional tutoring and training.
