The primary purpose of a communication management plan is to document the organization structure and methods of information collection, screening, formatting, and distribution. It also outlines understanding among team members regarding their actions and the defined processes necessary to facilitate communication among people, ideas, and information.
I have attached two sample communication templates I currently use. I am an IT Sr Program Manager at CDC in Atlanta. I am often involved in a number of strategic efforts. Let me know if you need more help but you should be able to use both of these templates as outlines for yours. Remember, when creating a communication plan you must address the following:
1. WHO do you need to talk to?
2. WHY are you talking to them?
3. WHAT do you need them to know?
4. WHEN do you need them to know?
5. HOW do you plan to tell (communicate) to them?
6. WHERE do you tell them (medium you will use to communicate)?
