Determining Graphs for Data

Please help, we are using average and I need to change my graph from percentages, however, it just goes into chaos when I try to adjust it to the numbers the graph I need. Thank you.

1.00- 1.5 = (1) = Significantly above
1.6 - 2.5 = (2) = Consistently better
2.6 - 3.5 = (3) = Consistently Achieve
3.6 - 4.5 = (4) = Not consistently achieve
4.6 - 5.0 = (5) = Significantly below

Please help determine which is the best graph to use for the following data. Below is an example of individuals scores for a month. Should they be converted or can excel convert and plot at the same time? (For example, 2.0 will be a score of 2.)

Jack 2.0 = 2
Kim 1.6 = 2
Tim 1.5 = 1
Dave 3.4 = 3
Paul 3.1 = 3
Saul 3.1 = 3
April 4.0 = 4
Sue 2.3 = 2
Rain 2.6 = 3

Next question:

For team scores would I use for Consistently Achieve and consistently better 3.5 and 2.6 or is it 3.5 and 2.5 based on the scale I type above. Further, do you suggest a scatter or line graph for this one. Lastly, can you give me step by step how to put this on my computer.
