In Cognitive Behavioral Therapy (CBT) the case formulation is based on the Cognitive Model of emotional disorders, first developed in detail by Aaron T. Beck. Case formulation focuses on negative automatic thoughts and the subsequent dysfunctional behaviors, emotions and somatic symptoms. Deeper case formulations can include underlying cognitions regarding core beliefs and assumptions that make up the schema of a client's world. Case formulation guides the structure and treatment approach, including choice and timing of interventions as symptoms are prioritized and unified in a treatment plan. The case formulation needs to be grounded in empirical research, be open to disconfirmation, economical in its approach, and easily understood by the client. The therapist must be self aware and not program in personal bias and cultural influences. On the other side, the therapist cannot be overly accepting of supposed accuracy of a client's reported judgements, and must rely on ongoing mediating psychological assessments as the treatment progresses.
