1. Suppose that an airliner is parked at the gate and being serviced. The flight crew and passengers are not on board. The catering truck is mis-positioned and as it is elevated it damages the fuselage skin near the right rear door. There are no injuries, but the damage requires repair. Technically this is not an aircraft accident because there was no one on board intending to fly. It isn't even an industrial accident (OSHA) because there were no injuries. Suppose you are the director of safety for this airline.

This event will be called a general accident. It is an accident because the catering truck was not mis-positioned on purpose, but it has damaged the fuselage skin near the right rear door. This damage needs to be repaired and so it is an accident.

Will it be reported? Investigated? By whom?
As I am the director of safety, it will be reported to the vice-president of the department. It will be investigated by one of my officers who will ascertain how the damage will compromise the safety of the aircraft and how such an accident can be prevented in future.

What are the direct (uninsured) costs of this event?
The direct costs of the event are cost of repairing the damage and the downtime the aircraft suffers because of the repair time.

What are the possible indirect (uninsured) costs of this event?
The possible indirect costs of this event will be related to flight delays because of the non-availability of aircraft and the loss of reputation because of these delays.

In addition, I will maintain all safety and accident records I will lead a companywide team of safety officers. .

Conduct, coordinate, and review safety audits, accident investigations and trends in safety information. Provide safety training program throughout the company. Ensure compliance with the safety training program throughout the company. Revise and update safety & environmental manual relating to safety. Organize the safety award program and assist with investigations of accidents and incidents involving the organization. .
