Hi and thank you for using Brainmass. The solution below should get you started. As you know, I cannot complete your homework for you, but I can show you how to do it, and guide you to achieve it. It appears that what you are being asked is complex, but this isn't the case - all that you need to know is that you need to give a reason for the survey items, your plan to collect the data, how you plan to analyze it and the problems/challenges of undertaking it. Now, I suggest using this outline:

1. Purpose of survey - 150 words
2. 10 survey items - samples (should be about 300 words)
3. Rationale for the items, data collection and analysis - 150 words
4. Issues - 100 words

This should be around 700 words but due to the fact that you have the survey items, this is more than enough.
