Printed questionnaires:
With spreadsheets and word processors since data could be entered with relative ease, with or without verification.

For some printed questionnaires, if it is being designed in a form of multiple-choice, there is a technology which allows MC-forms to be read, just like when you doing the SAT-test. These answers can be "read" by the computer because the soft pencil lead that filled in the bubbles or squares conducted electricity between matched pairs of minute electrodes. Even more recently, the marked forms could be scanned optically too.

Telephone survey:
Can manually enter the answer/choice into the computer at the same time the interviewee answered the questions. The interviewer can produce the relevant forms in the computer before hand.

Bank checks:
Should do manually as to ensure the accuracy of the amount and the check numbers, and also serves as a control.

Retail tags:
Scanner/bar code reader, for scanning the bar code - The use of bar coding has been growing dramatically over the last 15 years. With the adoption of UPC as the standard for retail grocery stores in the late 70's, bar codes have become an everyday experience for most people. Bar codes are a fast, easy, and accurate data entry method. The correct use of bar codes can decrease employee time required and increase an organization's efficiency.

Long documents:
Since it is a long document, scanning directly into the computer would be a good choice. Since it save time and also to prevent situation where incomplete input of data, so to increase accuracy.
