1. What were the problems experienced?

The audit identified four problems experienced by UW systems. Three of them represent significant deficiency in the internal control systems at UW System:

Access to the Human Resource System: Significant deficiency in overall HRS security review process resulting in inappropriate access to the system. For example, the audit identified 22 users with incompatible access that does not provide for proper separation of duties. Further, 21 of these users have these capabilities on a system-wide level, which allows them to make changes at any of the UW institutions.
